Prior to the first day of school, Lake Harriet UM Preschool must have a recently completed copy of the following forms on file for each enrolled child:
- Developmental History
- Emergency Data
- Family Information
- Health Care Summary
- Hand Sanitizer Permission
- Immunization Record
- Release
If you need another copy, you may download and print the form as a pdf file by clicking on the name of the form you need. Please provide us with an updated form if any of the information changes during the school year.
